Pre-Existing Conditions: You agree that Dot’s Spot Pet Grooming LLC is not responsible for any known or unknown pre-existing or degenerative medical conditions or the aggravation of such conditions. Such as but not limited to, heart disease, arthritis, intervertebral disc disease (IVDD), cruciate ligament tears, brachycephalic syndrome, infections, or any other medical problem that may be affected by the grooming process. You understand that shaving a matted pet may reveal pre-existing conditions that were unable to be identified prior to shaving and that Dot’s Spot Pet Grooming LLC is not responsible for issues that are revealed by or arise due to shaving a matted pet. This includes but is not limited to, skin irritations, poor hair regrowth, coat color changes, nicks to moles or skin growths, aural hematomas, and odd behaviors. In the even mats are found, you authorize the groomer to shave the matted area(s).
Vaccinations: Dot’s Spot Pet Grooming LLC requires proof that your pet’s rabies vaccine is up to date. We recommend that your pet is vaccinated against other infectious conditions including DHPP, leptospirosis, Bordetella, and canine influenza. It is still possible for a vaccinated pet to become ill despite being vaccinated. You agree that you will not hold Dot’s Spot Pet Grooming LLC liable in the event that your pet becomes ill with an infectious condition during or after the service.
Customer Information/Indemnification: In the event your pet is involved in an incident (e.g. biting a person), you hereby authorize Dot’s Spot Pet Grooming LLC to release your name and contact information to any party involved in the incident and/or the appropriate governmental authority. You agree to defend and indemnify Dot’s Spot Pet Grooming LLC for any claims, damages, or cost arising out of or related to any incident.
Photo Release: We may take a photo of your pet while your pet is receiving services. You hereby grant Dot’s Spot Pet Grooming LLC the perpetual, irrevocable, royalty-free right and license to publish, distribute, adapt, modify, or otherwise use the photo or any portion thereof, in any manner for any commercial or non-commercial purpose without notice to you/your review or approval.
Allergic/Adverse Reactions: You agree that Dot’s Spot Pet Grooming LLC is not responsible for allergic reactions resulting from the manufacturer-recommended usage of any products. Although a pet may experience an allergic reaction to grooming products at any time, flea and tick products are associated with a higher incidence of reactions. Please consult your veterinarian prior to having your pet treated if you have any questions concerning your pet’s sensitivity to such treatments.
Safety/Dog Behavior: Pet parents must inform us prior to grooming if your pet has aggressive tendencies or has bitten anyone. Dot’s Spot Pet Grooming LLC will not accept any aggressive dog. If false representations about the pet’s behavior were made when asked during booking of an appointment, we will discontinue services and the client will still be responsible for the full grooming charge. Clients will be liable for any bites or any property damage caused by their pet(s). For Groomer’s safety as well as your pets, Dot’s Spot Pet Grooming LLC has the right to refuse service in the event of a pet that cannot be handled safely. FOR OVERLY AGGRESSIVE OR OVERLY STRESSED PETS, DOT’S SPOT PET GROOMING LLC WILL NOT BE ABLE TO MAINTAIN YOU AS A CLIENT.
Pricing/Consultation: All prices are subject to change due to the condition of the coat, tightness of the mats, difficulty of the haircut, temperament of the pet, and/or the additional flea and tick charge. By signing this waiver you agree to pay the charges presented at the time of service completion.
Cancellation Policy: Dot’s Spot Pet Grooming LLC respectfully requests a 24 hour cancellation notice. Any cancellation made less than 24 hours will result in a 50% service cost fee. If cancellations are consistent (2 or more) with your account, you will be taken off a standing appointment schedule and there will be a 50% non refundable deposit for each appointment scheduled in the future.
Matted Pet Release:Dogs with severely matted coats require extra attention. Dot’s Spot Pet Grooming LLC will not cause serious or undue stress to pets by de-matting. Mats can be difficult to remove, and may require the pet to be shaved. Removing a heavily matted coat can cause nicks, cuts, or abrasions. Heavy matting can also trap moisture and urine near the pet’s skin, allowing mold, fungus or bacteria to grow, and or revealing skin irritations that existed prior to the grooming process. After-effects of the mat removal can include itchiness, redness, self-inflicted irritations or abrasions, and failure of hair to regrow. In some cases, pets may also exhibit brief behavioral changes. I hereby release Dot’s Spot Pet Grooming LLC from any liability associated with any and all injuries or medical problems that may be uncovered and or occur during the stripping and de-matting process. Should my pet need veterinarian care after or during the process of de-matting, I agree to pay any and all veterinarian fees.
Agreement: By filling out this waiver, you agree that all information you provided is correct and that it is your responsibility to advise. Dot’s Spot Pet Grooming LLC of any changes to the health or behavior of your pet. Client affirms they are the rightful legal owner, or care giver to the pet for which services are rendered.
